REQUIRED INFORMATION AND DOCUMENTS IN THE CASE OF LOSS
 

When domestic transportation is an aforementioned;
  • Insured/insurant’ declaration regarding the damaged commodity
  • Original policy and its copy   
  • Commodity’s invoice original and copy,
  • Delivery note’s original and copy,
  • Freight invoice’s copy,
  • The minutes of accident / breath test (if there is a traffic accident),
  • Photographs regarding the damage, official reports made by authorities
  • Damage assessment report (truck, articulated lorry’s license tag and driver’s signature)
  • CEASE AND DESIST LETTER sent by insured/insurant to carrier through notary,
  • Certified Resolution For Signatory Authority
  • Photographs of damaged commodity,
  • Insured’s bank information,
  • Other additional documents that insurer considers they necessary.
REQUIRED INFORMATION AND DOCUMENTS IN THE CASE OF DAMAGES

When international transportation is an aforementioned;
  • Insured/insurant’ declaration regarding the damaged commodity
  • Original policy and its copy
  • Commodity’s invoice original and copy,
  • In sea freights, bill of lading, in international road transport, bill of carriage or CMR, in airline transportation, airway bill, in railway transportation, railway bill,
  • Bill of entry (importation)
  • Customs declaration form (exportation)
  • Damage assessment report (included carrier’s signature)
  • If any, official reports that prepared by Official Authorities
  • Certificate of weight,
  • Contract of carriage, freight invoice,
  • CEASE AND DESIST LETTER sent by insured/insurant to carrier through notary,
  • Photographs of damaged commodity,
  • Insured’s bank information,
  • Other additional documents that insurer considers they necessary.

  • Owner’s letter or invoice that consider carrier as responsible within the scope of statutory liability,
  • If any, Fright Insurance’s copy or owner’s declaration regarding the having not commodity policy,
  • Insured’s bank information,
  • Other additional documents that insurer considers they necessary.
